Insurance and Risk Management events

Each year UOIT plans, sponsors or hosts a large number of events. To assist in evaluating risk potential and to provide suitable insurance for the institution, students and staff, an approval process has been developed. All events (field trips, on-campus and off-campus) must follow the approval process and complete the applicable event form. A brief outline follows with the required forms.

How are day-to-day activities affected by insurance?

Our insurance requires us to document the approval in advance all activities or events, whether they are on- or off-campus.

  • By having the activity/event approved, we eliminate exposure to UOIT and the individuals organizing the activity/event.
  • Failure to have an activity/event approved may result in personal liability.

On- and off-campus events

You require:

  • Approved on- or off-campus event form (original to Risk Management, seven days prior).
  • Once approved, distribute copies as noted on form.
  • When a field trip involves an outside third party, we require a Certificate of Insurance from that party with UOIT added as an additional insured.
  • Only UOIT students can participate.
  • Out-of-province/country - please contact Risk Management.

Field trips within the province

You require:

  • Approved Field trip form (original to Risk Manager, seven days prior)
  • Health information form
  • Checklist

Field trips outside of the province

You need:

  • Approved Field trip form (original to Risk Manager, seven days prior)
  • Health information form
  • Checklist
  • Informed consent (subject to the event, consent signed at least seven days prior to the day of the event)
